Embalming Authorization Form

  • EMBALMING DISCLOSURE

    Except in certain cases, embalming is not required by law. Embalming may be necessary, however, if you select certain funeral arrangements, such as a funeral with a viewing. If you do not want embalming, you usually have the right to choose an arrangement that does not require you to pay for it. Embalming is required by Minnesota Statute 149A.9l, Subd. 3 in the following circumstances:

  • By signing below, I acknowledge that I have read and understand the above disclosure indicating when embalming is required by law, and, to the best of my knowledge, I have the authority to grant permission for the embalming of the named decedent.
